A 76 year-old man presented with back pain anorexia and cachexia. CT scan confirmed a pancreatic mass with liver metastasis. Liver biopsy confirmed a neuroendocrine tumour. Because of left ventricular failure he was unsuitable for tumour reduction therapy.
